Intake Forms & Kiosk

Send digital intake forms and let patients self-check-in on a kiosk.

For: Front desk

Intake Forms collect demographics, history, consents and HIPAA acknowledgements digitally before the visit. Completed forms arrive in the Intake Inbox for the front desk to review and attach to the chart.

Kiosk mode
  1. 1Open Kiosk mode on a tablet at the front desk.
  2. 2Patients confirm their identity and complete or update their intake.
  3. 3Submissions route to the Intake Inbox for staff verification.
Tip:Kiosk and intake displays also respect the clinic's 9 AM–8 PM Central hours.
